This form, right here, is pretty much what you will receive if you purchased a plan from the marketplace around mid-February. It states the following: your personal information, the number of people covered in the plan, the amount you paid for your insurance, and the subsidy amount they gave you. Why is this important? This is important because you will need to turn this form into your tax agent when you go to file your taxes. It's basically a way for the government and your health insurance to verify the information. If you don't receive the 1095-A form in the mail, don't worry. You can simply log on to your marketplace account and find it under the tax form tab. If you don't have access to your marketplace account, you can call Hawker Dug Up directly or contact your agent.
