Most people will receive the 1095, but you may get the 1095a or b instead. You'll need your 1095 to complete your 2015 tax return. Federal law requires your employer to fill it out and provide it to you and the IRS. That makes it super important for tax purposes. The form reports whether your employer offered you health coverage in 2015, if the coverage was affordable according to Affordable Care Act standards, and some details around the cost of coverage they offered. It will supplement, not replace your W-2, which reports how much you made and paid in taxes. Form 1095-C has three parts. Part one identifies you, the employee, and your employer. Part 2 indicates whether your employer offered medical coverage for the whole year or certain months only. It also states the lowest-cost plan that was available during that time. Part 3 lists people covered on your plan. Remember, you may not be able to accurately file your 2015 tax return without your 1095 form, so don't lose it.
