Hello there, Charles here with Express IRS Forms. Our topic today is the IRS Form 1094. The Form 1094 is filed either as a 1094-B or a 1094-C, depending on the 1095 form that you file with it. You see, Form 1094 is the transmittal document containing a summary of your 1095 forms. This means that if you paper file your ACA documents, you'll need to file either a 1094-B or a 1094-C with the IRS as well. On it, you will need to include your identifying information like your name, EIN, and address. You'll also need to include the total number of 1095 forms that you're sending and the designated government entity or aggregated group information if applicable.
